What’s Included with the Initial Consultation?
Dr. Trendafilova will review your medical history and current issues prior to your appointment, so that your treatment plan will be comprehensive and specific to you.
You will submit current medical issues you would like addressed at time of scheduling your appointment.
After your appointment is scheduled, Dr. Trendafilova will request any relevant information be sent over as soon as you can, prior to your appointment, such as primary care doctor’s note, specialist notes, labs, imaging, etc.
You will have a 60 minute un-rushed consultation with Dr. Trendafilova.
This Initial Consultation fee includes time for Dr. Trendafilova to submit for any labs, prescriptions and prior-authorizations you will need.
Includes follow-up emails or phone calls with your doctors and yourself in between appointments, as required, for your treatment plan. Text the doctor directly if you prefer.

Appointment & Billing Details
Cost: The cost of a new patient consultation is $435, and the cost of a follow-up visit is $435. The initial consultation fee includes all continued communication and any necessary paperwork, such as prior authorizations, for the first 30 days after the initial visit. There are no additional fees.
Telehealth: Patients may schedule follow-up appointments via telehealth, if they are physically located in the state of Illinois at the time of the appointment. The doctor requests initial consultations to take place in person.
Medicare: Dr. Trendafilova has “opted out” of Medicare for her services. Some supplemental insurance carriers may provide some reimbursement for patients seen by a non-Medicare physician. You can still use your insurance as you normally would, to pay for labs, medications, imaging and referrals that she orders.
Commercial Insurance Plans: Dr. Trendafilova is considered out-of-network with insurance carriers. After your visit, the doctor will provide you with a “superbill” which you can submit to your insurance for reimbursement. Please check with your insurance carrier regarding your out-of-network benefits. You can still use your insurance as you normally would, to pay for labs, medications, imaging and referrals that she orders.
